Summit Academy Referral
*Note* - Summit Academy admits students through School District referral ONLY. If you are the parent/guardian of a student in Nebraska and you feel your student would benefit from our services, please discuss our services with your school administrator.
Step 1 - Screening - If you are a school administrator and have a student you believe could benefit from Summit Academy, complete and submit this screening form.
Step 2 - Summit Academy will review the screening form. We will then be in contact with you to discuss whether or not the student may be a fit for our program.
Step 3 - If Summit Academy staff feels the student might be a good fit, we will guide school administration through the contracting process.
Step 4 - A Behavior Specialist with ESU 9 will work with the school to observe the student, offer recommendations, and assist the school in the Summit Academy Application process.
Step 5 - Summit Academy Advisory Team will evaluate the application and determine admission status.
Step 6 - The student will begin a 30 day trial period during which testing and evaluations may be completed, behavior plans finalized, and Tier placement determined.
